What is an allergist? An allergist is a physician with two extra years of training
who is skilled and experienced in the evaluation and treatment of allergic diseases
such as asthma, hay fever, eczema, food and insect allergy and related
immune deficiency diseases.

Why see an Allergist? Many people suffer from related medical conditions not realizing
the exaggerations of their symptoms is the result of their being allergic to air born allergies,
foods, pets, insects.  These allergens can be tested to determine the level of the allergy.
Then the allergy can be treated to reduce the symptoms that require daily medication.

We ask that NO PERFUME be worn to our office as this can cause other patients
to have breathing difficulty

What forms of payment do you accept? We accept cash, personal checks and most
major credit cards such as Visa, Master Card and Discover. Payment is made when you
check in at each visit.

What will I need for my first appointment? You will need a photo ID (Driver license,
Military ID, Florida ID Card) your current insurance card, completed forms which are
listed under "FORMS" in our website, all current medications including inhaled, topical,
alternative,herbal, and vitamin supplements, any x-ray report or other relevant
information. If you need a referral/authorization you need to contact your physician
to obtain the referral prior to being seen. All co-pays/deductibles are due at the time
of the office visit.

How long will my first appointment take? Most first office visit can involve skin
testing and pulmonary function test  so the could take about 2 hours. Were comfortable
clothing as most skin testing is preformed on the back (see Skin Test Video)  bring a
light jacket as the office is Cool.

Patients under 18 years of age must have a Parent or Legal Guardian (court papers)
with then at their first appointment.(photo ID-Drives license, Military ID, Florida ID card)
The Permission to involve others form can be filled out and signed so that others can
bring the child to future appointments.

Can I be seen if I'm sick on the same day?  We make every attempt to see our sick
patients on the same day. All of the offices and all providers have "sick" appointment
available on a daily basis on a first come first serve basis.

How do I obtain medication refills? All medications refills are obtained at the time of
the office visit. Just notify the provider at your visit if you will need 30 days or 90 day
supply. The amount of refills will last till your 6month follow up. We will not refill any
medications if you have not been seen in 6 months.

How can I cancel or reschedule my appointment? Please call the office where the
appointment was made 24 hours before your appointment or earlier to avoid a
cancellation fee of $25. (Missed appointments are charged a$25 fee that must be paid
before the next visit)

What days are you closed? We closed everyday for lunch between 1pm-2pm. We are
also closed for all major public holidays January 1st, Memorial Day, July 4th, Labor Day,
Thanksgiving and the Friday after, also Christmas Day.

Allergy injections are not given between Christmas and New Year.

Can I e-mail the providers? If you pre-registered with the RelayHealth
website www.relayhealth.com you will be able to send your doctor e-mails
regarding non urgent medical issues.
