Our staff is friendly and always ready to help with scheduling, insurance or anything else you may need during your time with us. Our doctors will take extra care to listen to your concerns, answer all your questions and thoroughly explain your condition and treatment options.

New Patient Forms:

If you have made your initial appointment, you can preregister by clicking on the links below to download our New Patient Forms. Please print and complete the forms prior to your arrival. This will help to speed up the new patient registration process and get you in to see the doctor faster!

Appointment information:

To schedule an appointment with Allergy Asthma Specialists please call us at 407-339-3002. If at any time you are unable to make your appointment, please notify us at least 24 business hours in advance to avoid a $30.00 No Show/Cancellation fee. We would be glad to reschedule your appointment at a more favorable time, if necessary. We greatly appreciate your time and consideration and look forward to seeing you.

We ask that NO PERFUME be worn to our office as this can cause other patients to have difficulty breathing.
For your first appointment you will need a photo ID (Driver license, Military ID, Florida ID Card), your current insurance card, completed forms which are listed under “FORMS” in our website, all currrent medications including inhaled, topical, alternative, herbal and vitamin supplements, any x-ray report or other relevant information. Most first office visits can involve skin testing and pulmonary function testing, so it may take about 2 hours. Wear comfortable clothing as most skin testing is performed on the back (see Skin Test Video) and bring a light jacket as the office is Cool.

We make every attempt to see our sick patients on the same day. All of the offices and all providers have “sick” appointment availability on a daily basis on a first come first serve basis.

Prescriptions:

Please call our office for all prescription refills ONE WEEK PRIOR to the end of your medication to prevent any lapse in coverage.

Insurance:

We accept most insurance please call us to verify whether or not we are participating with a particular insurance company.

Billing and Financial Responsibility:

Please review our financial policy or call our office for assistance with any billing concerns. We will be happy to assist you.
We accept cash, personal checks and most major credit cards such as Visa, Master Card, American Express and Discover Credit Cards. Payment is made when you check in at each visit.
